How to Request Your Records

We offer several convenient options for submitting a medical records request.

Submission Options

What to Include in Your Request

  • Your full legal name and date of birth
  • Dates of service for the records requested
  • Type of records needed (visit notes, imaging reports, lab results, operative reports, etc.)
  • Name and mailing address of the person or facility to receive the records
  • Signed HIPAA Authorization Form (see below)
  • Copy of a valid government-issued photo ID

How It Works

No username or password required

Enter your email at the link above and you’ll receive a secure login link — no credentials to remember. Click “Submit Request,” follow the prompts, and you’ll receive an email as soon as your records are ready to download.

Processing Time

Most requests processed within 8 business hours

One to two business days after submitting your request, your records will be delivered electronically to your inbox. You can check the status of your request any time inside HealthMark Request Manager.

HIPAA Authorization

Release of Records to a Third Party

A signed HIPAA Authorization Form is required whenever records are to be released to a third party, including other healthcare providers, attorneys, insurance carriers, or any person other than the patient.

The HIPAA Authorization Form is available at the front desk of any of our clinic locations, through the Patient Portal, or by calling our office during business hours.

Patients accessing their own records directly through the Patient Portal do not need to submit a separate HIPAA Authorization Form. The portal provides secure, on-demand access to your visit summaries, lab results, imaging reports, and more.

Fees
Maryland Law Applies
Maryland law permits a reasonable charge for medical record production. Any applicable fees will be communicated to you prior to release.
Imaging (CD/DVD)
Available Upon Request
Copies of imaging studies on disc or digital media are available. Standard fees apply.
